Hope Home Support Services Service Delivery

The person-centred approach is at the heart of all services provided by Hope Home Support Services.

We place the person first, and focus on providing support to achieve their aspirations, and to be tailored to their needs and unique circumstances.

We work collaboratively with participants and their families as appropriate and practicable, uphold their legal and human rights, and respect their culture, diversity, values and beliefs.

If at any time we feel we are not an appropriate service provider for a participant, we will discuss this and assist the participant to access more appropriate services.

At Hope Home Support Services we believe that ‘communication’ is the key component to our client relationship with each participant. We will aim to communicate with participants in the language and mode that they identify and best understand.

We will work with the participant and identify any risks associated with the delivery of supports and identify mitigation strategies that will allow participants to exercise dignity and to take reasonable risks as they see fit. We will develop and review support plans that will reflect the participant’s needs, goals, strengths, requirements, and preferences.

Hope Home Support Services works to ensure the delivery of high-quality supports to participants who choose to purchase them from us. This means we comply with the NDIS Code of Conduct, are committed to ongoing continuous improvement activities, to give participants the benefit of the best quality supports we can provide in a safe environment.

Start Using Our Service Now

Hope Home Support Services include:

the provision of transition supports for people seeking a place to live, which suitably caters to their type and level of needs;

such as meal preparation and delivery, house or yard maintenance, cleaning and laundry;

Assistance with daily personal activities to enable people to live as autonomously as possible – such as personal hygiene, dressing, medication assistance; financial management, communication skills and lifestyle mentoring;

Assistance with daily tasks in group or shared living arrangements to help individuals to live as autonomously as possible;

Assistance with life stage transitions, including mentoring, peer support, individual skill development, and decision making to support people to make transitions on their own terms and in a way that meets their goals and aspirations;

including developmental-focused activities and training to enhance the person’s capacity to live as independent an everyday life as possible;

such as social outings and group and social activities on Hope Home Support Services’ premises;

includes activities outside the scope of ‘Participation in the Community’;

including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport and building confidence and social skills;
Hope Home Support Services and the National Disability Insurance Scheme (NDIS).

Support Independent Living

Supported Independent Living (SIL) is a service Hope Home Support Services (HHSS) provides to people living with disability, who wish to be supported to live either independently in their own home or in shared accommodation.
